You can expect your appointment time to last approximately 30 minutes. In order for us to be completely prepared to meet your needs, it will be helpful for you to print and complete our online forms and bring them with you to your appointment:-

- Patient-Family Agreement (ENG)
- Patient-Family Agreement (SPN)
- Case History (ENG)
- Case History (SPN)
- Insurance Information/Verification Sheet

These materials will assist us in clarifying information prior to your evaluation date. This appointment has been exclusively reserved for you. Please call us if you are unable to keep your appointment.

HELPFUL CHECKLIST

- Check with your insurance company to verify your coverage.
- Obtain a prescription from the physician and provide it to us prior to/or on the day of the evaluation
- Complete the Patient-Family Agreement, Case History, & Patient Information Sheet and bring them with you to the evaluation.
- Bring your insurance card.
